The truck runs out of fuel & the gauge reads 1/4 tank
I disconnected the batteries to see if it would reset something but it didnt.

bet it is the pickup/sender in the tank. they are pricy and they dont even have a pump on them. you need to drop the tank and inspect the sender.

About $400.00 or so. depends on how much the dealer wants to mark it up. Unless you have any chafed wiring or a bent baffle in the tank causing the float rod to bind then i would assume the sender

Do you have dual tanks, I had a Ford E150 that hat dual tanks and my gauge showed 1/2 full turned out to be a blown fuse. The second tank was empty, but the gauge showed the half tank when I was really running on the empty one. Always check the easy thinks first

I only have 1 tank. The thing is when it runs out of fuel the gauge reads 1/4 tank, then if I fill it up it will only take around 15gal. Is it possible there's still a 1/4 tank of fuel left and it still runs out?

Thanks for all the help.

having a blown fuse is unlikely. the circuit that carries the info to the instrument cluster is of variable resistance so no load is carried on that circuit so no need for a fuse. your truck (standard) ususally has a 29 gallon tank. at empty i can get 25 in my truck so i would assume at 1/4 tank you could fill from 20-15 in it. the next problem is the pickup screens and tank sump. the sump on the botton of the pickup assy can get brittle and break off making the pickup assy much shorter and not allowing it to pick up any more fuel below a certain ammount. (about 3 inches up) you need to remove the tank and inspect the pickup assy. clean all dirt and screens in the pickup. but i bet the rubber sump fell off the bottom.

Now that sounds more like whats going on, Thanks
I will check it & get back with ya.
Have a great weekend.
